The Employer Care Program, provides a convenient, no cost option for acute, preventive, and wellness focused primary care services for employees and dependents of our partner employer clients ranging from preventive care, chronic disease management, medication therapy management, and healthcare coordination. Healthcare coordination can help you get healthy and stay healthy in all aspects of your life. The services provided can be shared with your personal primary care physician.
