Welcome to the Oak Creek-Franklin Health & Wellness Center (HWC) providing quality healthcare delivered by Ascension Wisconsin
The HWC is available to all employees of the School District enrolled in the medical plan. Any spouse, domestic partner or dependent children aged 2 or older who are covered under the employee’s medical insurance through the School District can also use the health center.

The HWC offers in‐person and virtual appointments, so you can take advantage of the clinic wherever you are.

Services provided
Clinic Services
- Diagnose and treat primary and acute health care needs
- Manage chronic illness such diabetes, arthritis, high blood pressure, etc.
- Write prescriptions and dispense medications
- Screenings and preventive care
- Make referrals to your network provider for specialty medical appointments
- Lab work and vaccinations
- Conduct adult and pediatric (2+) physicals
- Pre‐employment physicals
- Acute care needs such as sore throat, ear and sinus infections, cold, flu, headaches, etc.
Wellness Incentive Program
Oak Creek-Franklin Joint School District staff members and spouses on the insurance plan are required to complete a Health Assessment AND biometric screening with their primary care provider at their annual visit to avoid additional insurance premium costs.
Requirements:
- Log onto the portal (https://ocfsdwellness.wellright.com/)
- Complete the Health Risk Assessment questionnaire within the portal.
- Complete your annual physical and biometrics with your primary care provider and upload your PCP form to the portal.
Important: If your spouse is participating in the program, they must also be registered for the Wellness platform to get credit for their biometrics and HRA completion.
Meet the care team

Cynthia Stasik, PA-C
Cynthia Stasik, PA-C, moved to Milwaukee to pursue her passion of medicine and received her Master's in Physician Assistant Studies from Marquette University. Originally from Chicago, she stayed in Milwaukee and has called it home for over 20 years.
Cynthia brings a warm and caring practicing style to her patients. She believes in establishing a partnership between herself and her patients with a focus on healthy lifestyles and medical management to help her patients achieve their health and wellness goals. Her experiences include Family Practice, Urgent Care, and Pain Management. She is also passionate in matters pertaining to Women's Health and Maternal Mental Health.
Outside of work, she enjoys reading, cooking, gardening, trying new restaurants, and spending time with family - her husband, young daughter, and their French bulldog.

Janet Kieslich, MSN, RN, FNP-BC
Janet has been in nursing for over 30 years. Providing patient care to the entire family brings her joy, knowing she can truly make a difference in people’s overall health. She strives to help people improve their quality of life, achieve their health and wellness goals and support their best possible self in body, mind and spirit. Janet enjoys developing special connections with adolescent patients helping them realize the importance of healthy lifestyle decisions which carry over into adulthood. She takes interest in peri and post menopause, weight reduction, exercise, gut microbiome, using natural remedies first and focuses on chronic disease prevention.
Janet is an ANCC certified family nurse practitioner with over 20 years’ experience. She received her BSN at the University of Wisconsin - Madison. She completed her MSN at Marquette University for her Adult NP certification and obtained a post master’s certificate from The George Washington University for her Family NP certification.
On the weekends, Janet volunteers as a substitute Sunday School teacher at her church. She is married and blessed with raising two sons. Janet is an avid exercise enthusiast enjoying many sports such as pickleball, golf, volleyball, skiing, hiking, fishing and billiards. She is also certified through the American Council on Exercise (ACE) as a group fitness instructor and teaches various yoga classes.

Jeaneen Hughlett, MA
Jeaneen Hughlett was born and raised in Milwaukee and has been working in the medical field in the area for over 20 years. She graduated from Kaplan College with a Medical Assistant degree and is also a certified patient home care worker. Her previous medical experiences include working in cardiology, dermatology and urgent care clinics. She has learned and gathered a lot of experience working with various doctors, PAs and nurses in these fields. She is passionate about her role in healthcare and has known since childhood that this is something she has always wanted to do. She is a caring person, and it brings her happiness to know she has helped someone feel better. She is excited to continue her role at the Oak Creek Health and Wellness Center, working with the District's staff.
In her spare time, Jeaneen enjoys journaling, praying, listening to music, singing, reading books, bike riding, traveling and cooking. She also loves spending time with her four children - particularly watching sports with them. She also has a small yorkie, Mocha, that she loves to play with.
