The Catholic Diocese Health and Wellness Clinic, in partnership with Ascension Medical Group, works with you to help you achieve health and wellness goals by providing services ranging from preventative care, chronic disease management, medication therapy management, dietetics services and health coaching.
Catholic Diocese associates and their dependents who are enrolled the Catholic Diocese PPO Medical Plan, can use the Health and Wellness Clinic at no cost.

With top notch quality care, this extension of service is here for you and your family. Whether it is an urgent care need or a new medical condition, Employer Care sites can help. With 10 additional local locations, don’t go without care.
Employer Care locations offer the same high quality health care and convenience from Ascension St. Vincent Tri State Clinics. These clinics are equipped to care for you and your family with the most commonly used medications and vaccines. Some medications will be made available through free next day mail delivery or pick up from the Catholic Diocese Health and Wellness Center.
