The Center provides a convenient, low to no-cost option for acute, preventive, and wellness focused primary care services for all FCA employees and their families enrolled in the FCA Health Care Plan. Our goal is to be your trusted partner in health and wellness. We provide access to a broad range of services designed to meet your individual needs and work with you to achieve your goals to be a healthier you.
Through a team approach, we commit to more efficiently and effectively coordinating your care to help you avoid delays and unnecessary services. This includes communicating with your current physicians about your care at the Center, if you choose to do so.
Your care is between you and your doctor. It is never shared with your employer

Services Provided
Primary Care: Our experienced care team can help with the full range of your family’s everyday health needs, including:
- Preventive care, annual physical exams and health screenings for all ages
- Full pediatric services including well-child and sick visits
- Vaccinations for adults and children
- Management of conditions such as asthma, diabetes, high blood pressure and arthritis
- Diagnosis and treatment of illness and injuries
- Guidance on nutrition and fitness
- Medication management
- Referral to specialists as needed
Imaging: We provide x-ray services with easy scheduling. All exams are interpreted by Board Certified Radiologists.
Lab: Conveniently located inside our clinic, laboratory services are ready when you are, which makes bloodwork and other testing an easy process. We are available before and after your appointment or you can walk-in anytime during regular office hours.
Behavioral Health: Mental health is a critical component for overall wellness. Our Behavioral Health Counselor can help with anxiety, depression, relationship issues, grief counseling and more. Care is provided both in person and virtually.
Physical Therapy: Ascension Physical Therapy provides a wide range of outpatient therapy appointments to accommodate your work schedule. Our therapists deliver evidence based care that promotes healing, relieves aches and pains, and helps restore strength, flexibility, and overall well-being. We listen to you and start where you are, one step at a time.
Dietician and Athletic Trainer: Whether it is for weight loss or managing chronic conditions like diabetes, we have professionals on-site to help you meet your goals.

Important safety precautions
We are fully prepared to safely care for you and your family. Our commitment to compassionate, personalized care remains unchanged.
We continue to maintain strict precautions when you are in our care including:
- Cleaning and disinfecting -- Performing rigorous procedures to clean and disinfect in our sites of care.
- Wearing personal protective equipment (PPE) -- We continue to wear PPE (including masks) for the safety of our patients and caregivers when needed. We ask that all patients exhibiting symptoms of a contagious illness wear masks in our facilities for everyone’s safety. Read more about proper mask wearing here
