The Health & Wellness Centers provide a convenient, low cost option for acute, preventive and wellness focused primary care services for all School District employees and their families. The services provided can be shared with your personal primary care physician. This coordination can help you get healthy and stay healthy in all aspects of your life.

ANY patient who has not been seen by Kristi with Ascension is considered NEW at this clinic, and will need to schedule a one hour physical appointment prior to getting their medications refilled.

The Health Center Staff
The Health & Wellness Centers provide accessible health and wellness resources that improve the well-being of employees and their dependents. The Health Center staff provides high quality care to treat, reverse and prevent disease that match each individual’s needs. The onsite provider can serve as your primary care provider or will work in collaboration with your established primary care physician.

- Disease management (diabetes, cholesterol, blood pressure, etc.)
- Lifestyle/Health coaching
- Wellness education
- Primary care/Preventive services
- Routine annual physical exam (ages 2+)
- Vision screening
- Blood pressure
- Cholesterol
- Breast exam
- Pap test
- Prostate exam
- Flu shot
- Rectal exam
- Tetanus
- Physicals (annual physicals such as Well Woman/Man/child (ages 2+), sports)
- Nonāpreventive care
- Sore throat
- Ear & sinus infections
- Cold, flu, etc.
- Bladder infections
- Allergy care
- Headaches
- Minor injuries
- Minor procedures (removal of warts and minor wound care)
- Lab work and vaccinations (can complete lab draw with orders from outside provider)
- Referral to specialists (as needed)
- Coordination with outside providers
