The Atlas Health and Wellness Clinic, in partnership with Ascension Medical Group, works with you to help you achieve health and wellness goals by providing services ranging from preventative care, chronic disease management, medication therapy management, behavioral health services, dietetics services and health coaching.
Atlas associates and their dependents who are enrolled in an Atlas sponsored medical plan, can use the Atlas Health and Wellness Clinic at no cost.

Can’t make it to the onsite Atlas Health and Wellness Center? Not a problem. Ascension St. Vincent Tri State Clinics Employer Care locations are here for you. With top notch quality care, this extension of service is here for you and your family. Whether it is an urgent care need or a new medical condition, Employer Care sites can help. With 10 additional local locations, don’t go without care.
Employer Care locations offer the same high quality health care and convenience from Ascension St. Vincent Tri State Clinics. These clinics are equipped to care for you and your family with the most commonly used medications and vaccines. Some medications will be made available through free next day mail delivery or pick up from the Atlas Health and Wellness Center.

- Access the Medikeeper Health Portal.
- To access your health records, please follow the link for your Ascension Health Portal.
- Select Indiana > Ascension St. Vincent > Outpatient Portal.
- For patients who do not have login information, please visit a Tri State Clinic or call the office at 888-492-8722 with your personal email address. They will send you the information needed to access your patient profile.
